Job Duties Include:

   QUALIFICATIONS:

1.      Current licensure as a Registered Nurse or Therapist in the states of MA and NH.

2.      Registered Nurse BS preferred, or Therapist with two (2) years experience in community health/home health, Supervisory experience preferred.

3.      In depth understanding of Medicare Conditions of Participation and PPS, and case management.

4.      Demonstrated strong organizational, planning, problem solving, decision-making, leadership, and motivational skills.

5.      Demonstrated ability to communicate effectively in writing and verbally with people of varied backgrounds and levels of understanding.

Director of Clinical Services

Hospice Manager

Utilizing the nursing process, plans for coordinated hospice services for individuals and their families at our 10-bed Miriam Boyd Parlin Hospice Residence and/or Parmenter Hospice in the community.

Provides appropriate preventive, palliative and rehabilitative care through assessment, teaching, counseling and direct care to client and/or family members.

Coordinates care with other team members and attends Hospice Interdisciplinary Team meetings.

Supervises, schedules and directs the care provided at the Miriam Boyd Parlin Hospice Residence.

Coordinates admissions of hospice patients and fosters relationships between patients and facilities providing their care.

Establishes patients' eligibility for care, communicating with families and interacting with a wide range of staff members, care coordinators, case managers and physicians.

Supervises a staff of approximately 25.

 

Requirements:

The Performance Improvement Manager develops, implements and maintains a Performance Improvement Program including a system which monitors and measures indicators of operational performance, clinical outcomes and compliance with federal regulations and JCAHO standards; integrates the Opportunities for Improvement PI model into the organizational culture.

The Social Worker is responsible for providing psychosocial care to hospice beneficiaries and their families regarding economic, social and emotional needs. He or she will implement and coordinate all social service activities of the hospice program under the direction of the Hospice Medical Director.

Position qualifications & preferred skills and/or experience: Previous hospice experience is preferred. Experience with oncology, geriatric, and/or hospice individuals is desirable. Experience in a customer-service oriented environment is preferred. BSW degree required. MSW degree is desirable. Must be licensed as an LICSW. Per Diem Nurse Practitioner

In this per-visit role, the NP will conduct face to face encounter visits. Compensation is remunerated at a handsome rate per visit, with a minimum of 2 hours paid per patient visit.

Admission Liaison RN, North Shore Area ( W / E Baylor, Sat & Sun, 8a-8:30p)

Reporting to the Admission Manager, the Admissions/Liaison Nurse is responsible for patient assessments and admissions to HNS, ensuring continuity of care between the admitting process and transition to ongoing care by the hospice interdisciplinary team (IDT). In addition, the Admissions/Liaison Nurse is responsible for educating health care professionals, patients, and families in all settings regarding the benefits/appropriateness of hospice care. The Admissions/Liaison Nurse is responsible for being proactive in identifying new referral sources, increasing referrals from existing sources, and providing educational programs to facilities. The Admissions/Liaison nurse maintains good communication with HNS clinical staff in order to enhance hospice services, and facilitates communication between HNS and referral sources. Will be responsible for traveling in our South Tier which will cover anywhere from Wellesley, Waltham, Milton, Needham and Norwood to Quincy, Braintree, Boston, Newton and surrounding towns.

Qualifications: Current MA RN license in good standing. Minimum of three years of varied work experience as a professional nurse, preferably one of those years in an acute setting. Hospice nursing experience is highly desirable. One year additional work experience as a professional nurse in a homecare, hospital or skilled nursing facility with strong clinical assessment skills related to admission to or change in service. Must provide proof of current CPR.

Hospice Aides (Boston Metro):
Home care/LTC Team: Wellesley Office (Full-time, M-F, choose either 6a-2:30p or 7a-3:30p)

The Hospice House & Field Hospice Aides are vital members of the Hospice House interdisciplinary team. They are to care for patients in the Hospice House or in the field (field aides see patients either in homes or in Long Term Care or Assisted Living settings). The Hospice Aide functions under the direct supervision of the Hospice House or Clinical Manager with additional direction from the primary care nurse responsible for the particular patient.

Qualifications: High School Diploma or equivalent preferred. Certification of successful completion of a CNA or CNA/ Home Health Aide course approved by the Massachusetts Department of Public Health and in accordance with the requirements of the U.S. Department of Health and Human Services. Certification must be in good standing with the nurse aide registry and not have had a lapse in furnishing services within a 24 month period. In lieu of an HHA certificate, successful passing of hospice aide written test and skills assessment.

 

 

Chaplains (Boston Metro, PT, 20hrs & Per Diem)

As a member of the interdisciplinary team, the chaplain works under the supervision of the Clinical or the Hospice House Manager and is responsible for providing spiritual support to patients and families consistent with the patient/family's wishes and belief system. The chaplain is responsible for implementing and coordinating the provision of spiritual support to patients and families in accordance with the hospice plan of care.

Qualifications: Graduate degree preferred (Master's Divinity or Theology preferred), or Bachelor's degree with coursework from an accredited seminary or school of theology, with certification in hospital or pastoral ministry preferred. Minimum two years pastoral experience in a hospital or community ministry. Demonstrated ability to be open, flexible and ecumenical. Experience counseling patients and families who are dealing with terminal illness. Demonstrated understanding of hospice philosophy and principles. Demonstrated ability to be self-directed, flexible and cooperative in fulfilling role obligation, and ability to work effectively within an interdisciplinary team. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and in good working order.

 

 

Social Worker, Kaplan Family Hospice House (Per Diem, 4:30p-9p)

As a member of the Hospice House interdisciplinary team, the social worker reports to the Hospice House Manager , and is responsible for planning and providing supportive services to terminally ill patients and families. The social worker is responsible for identifying patient/family psychosocial needs, and for addressing those needs through direct service and/or consultation with other staff members in accordance with the hospice plan of care.

Qualifications: Master's degree in social work or related field preferred. Current Massachusetts social work licensure (LICSW preferred, LCSW accepted with appropriate work experience). Minimum three years counseling experience. Experience counseling terminally ill persons and their families; knowledge of the health care delivery system and community resources. Experience and training in leading support groups, and skill in the area of family psychosocial assessment. Demonstrated ability to assess and respond to the needs of patients and families in varied settings. Demonstrated understanding of hospice philosophy and principles.

Reporting to the Office Manager, the Receptionist performs a broad range of secretarial and administrative activities to facilitate the smooth and efficient operation of the office. The Receptionist is responsible for effective customer service and excellent telephone reception, as well as secretarial support for the organization.

Qualifications: Associate's degree in a business or secretarial curriculum preferred, high school degree required. Minimum one year experience, preferably in a health care setting. Clinical Director

Reporting to the Hospice Director, the Clinical Director will be responsible for the planning, organizing, coordination, and evaluation of all day-to-day aspects of Hospice patient care. The Clinical Director is a key member of the management team with a pivotal role in designing models of care and establishing standards of clinical excellence.

 

Duties and Responsibilities:

 

•  Plans, develops, directs, organizes, implements, and evaluates clinical services and activities. Participates in the identification and development of new Hospice programs and services.

•  Works closely with the Hospice Director and Medical Director to plan and implement policies and procedures relative to clinical operations and set standards for all performance objectives

•  Provides direct supervision of the staff RNs and make supervisory visits with patient caregivers as appropriate. Assigns Hospice patients to the appropriate staff, and assures that adequate staffing levels are anticipated and maintained.

•  Oversees home health aides, social workers, therapy providers (PT, OT, Speech), and complementary/alternative therapy provides (massage, Reiki, art, music) to ensure high quality care and services by the broader clinical team.

•  Educates clinical personnel regarding Hospice admission criteria, policies and procedures, patient care and Hospice philosophy. Ensures that all individuals with direct patient contact are educated in the Hospice philosophy.

•  Ensures that there are ongoing, robust staff development and skills training programs to help the clinical team deliver exceptional quality of care.

•  Coordinates care and functions as a liaison between all disciplines providing care. Leads and participates as an active member in the interdisciplinary group (IDG) meetings. Identifies need for staff education and assists in the development of in-service programs for the IDG.

•  Assures effective coordination between clinical and clerical staff so that paperwork necessary for payroll and billing is submitted accurately and according to established time frames.

Qualifications:

 

•  Previous clinical management experience in Hospice is required.

Hospice Nurse Case Manager

As a member of the interdisciplinary group (IDG), the Hospice Nurse Case Manager provides professional nursing care to the Hospice's patients. Identify patient/family needs and provide supportive care in accordance with the attending physician's orders, the IDG plan of care and the Hospice's policies and procedures. The Hospice Nurse Case Manager provides care under the direction of the attending physician, the Hospice Medical Director, the Clinical Director, and in compliance with the state's Registered Nurse Practice Act.

 

Position Duties and Responsibilities:

 

•  Assumes primary responsibility for a patient/family caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process. Provides Hospice nursing care based on systematic assessment focusing on pain control and symptom management.

•  Initiates communication with attending physicians, other Hospice staff members and other agencies as needed to coordinate care and use of resources for the patient/family.

•  Assesses the patient and develops an individualized and comprehensive plan of care in partnership with the Interdisciplinary group and in accordance with Hospice regulations/guidelines.

•  Maintains regular communication with the Clinical Director to review caseload and with the attending physician concerning patient/family caseload.

•  Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporates that data into the plan of care.

•  Maintains up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur in accordance with agency policy and Medicare regulations.

•  Accurately documents assessment findings, observations, interventions and evaluations pertaining to patient care management and services provided in accordance with Hospice policy and procedures on the day services are rendered. Submits documentation in a timely manner.

•  Teaches, supervises and counsels patients/families regarding physical care and comfort, and other problems related to the patient's terminal condition.

•  Provides guidance to patients/families to assist them in preparing for and coping with anticipated physical and psychological events throughout the dying process. Provides appropriate support at time of death and period of bereavement.

•  Meets regularly with Hospice nursing staff to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth.

•  Participates in IDG conference and facilitates discussion of issues from caseload for full staff discussion, consultation and evaluation.

•  Supervises the Hospice Aide and the aide plan of care at least every fourteen (14) days and documents such supervision. If concerns are noted, observes the aide onsite providing care. If concerns are ongoing, reports to the Clinical Director of the need to repeat a competency evaluation with aide.

•  Informs the Clinical Director of unusual or potentially problematic patient/family issues.

•  Shares in providing 24-hour, seven-day a week coverage (evening and weekend on-call) to patients/families.

•  Demonstrates working clinical knowledge and skills relative to hospice and end-of-life care, and a commitment to clinical excellence and ongoing training in the latest pain and symptom management interventions.

 

Qualifications:

 

•  Graduate from an accredited School of Nursing. BSN preferred.

•  Currently licensed in good standing to practice as a Registered Nurse in the State of Massachusetts.

•  Must have no less than six months experience in acute care setting as an RN prior to home care assignment, or experience as required by state regulations. Minimum of one year's experience as an RN required. Experience in Hospice is strongly preferred.

•  Demonstrated ability to assess and respond to the needs of patients and families in varied settings and to cope with emotional stress.

•  Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with patients and their families.

•  Must accept responsibility for maintaining clinical practice skills, learning and adhering to Hospice's policies and procedures on an on-going basis.

Volunteers are often referred to as “the heart of hospice”, and the experience of being a hospice volunteer can be profoundly meaningful and rewarding. Similarly, the connections made between volunteers and their assigned patients and families are an integral part of making every patient and family feel fully supported. Position Duties and Responsibilities:

 

•Plan and supervise the delivery of all volunteer services.

•  Develop volunteer policies and procedures consistent with the hospice program's mission, vision, and goals.

•  Recruit, screen, and select volunteers for the hospice program.

•  Design and supervise the orientation and training of volunteers regarding hospice philosophy, policies, and practice, including ongoing education.

•  Document successful completion of the orientation and training program for all volunteers.

•  Assign volunteers in consideration of the skills, knowledge and motivation of the individual and goals of the program and/or plan of care.

•  Plan and conduct volunteer periodic support meetings and ensure that volunteers in the field feel competent, supported, and well-trained.

•  Foster volunteers' continued skill development and spiritual growth.

•  Maintain record keeping for volunteer service and time ; monitor volunteer compliance with record keeping.

•  Actively participate in the Interdisciplinary Group meetings. Ensure that volunteers are utilized as an integral part of the Interdisciplinary Group meetings when appropriate.

•  Oversee statistical data collection to assure compliance with the Medicare Conditions of Participation for Hospice regarding utilization of volunteers.

•  Conduct scheduled evaluations of volunteers in the program.

 

Qualifications :

 

•  Experience in leading/coordinating volunteer groups required. Hospice experience preferred.

•  Ability to communicate effectively with physicians, caregivers and hospice patients and families required. Public speaking experience preferred.

•  Bachelor's Degree- Human Relations or similar area preferred.

•  Emotional maturity, sensitivity, human relations skills, successful resolution of losses in own life, and good interpersonal skills.

Reporting to the Admission Manager, the Admissions/Liaison Nurse is responsible for patient assessments and admissions to HNS, ensuring continuity of care between the admitting process and transition to ongoing care by the hospice interdisciplinary team (IDT). In addition, the Admissions/Liaison Nurse is responsible for educating health care professionals, patients, and families in all settings regarding the benefits/appropriateness of hospice care. The Admissions/Liaison Nurse is responsible for being proactive in identifying new referral sources, increasing referrals from existing sources, and providing educational programs to facilities. The Admissions/Liaison nurse maintains good communication with HNS clinical staff in order to enhance hospice services, and facilitates communication between HNS and referral sources. Will be responsible for traveling in our South Tier which will cover anywhere from Wellesley, Waltham, Milton, Needham and Norwood to Quincy, Braintree, Boston, Newton and surrounding towns.

The Hospice House & Field Hospice Aides are vital members of the Hospice House interdisciplinary team. They are to care for patients in the Hospice House or in the field (field aides see patients either in homes or in Long Term Care or Assisted Living settings). The Hospice Aide functions under the direct supervision of the Hospice House or Clinical Manager with additional direction from the primary care nurse responsible for the particular patient.

Qualifications: High School Diploma or equivalent preferred. Certification of successful completion of a CNA or CNA/ Home Health Aide course approved by the Massachusetts Department of Public Health and in accordance with the requirements of the U.S. Department of Health and Human Services. Certification must be in good standing with the nurse aide registry and not have had a lapse in furnishing services within a 24 month period. In lieu of an HHA certificate, successful passing of hospice aide written test and skills assessment.

Chaplains (North Shore, PT, 20hrs)
As a member of the interdisciplinary team, the chaplain works under the supervision of the Clinical or the Hospice House Manager and is responsible for providing spiritual support to patients and families consistent with the patient/family's wishes and belief system. The chaplain is responsible for implementing and coordinating the provision of spiritual support to patients and families in accordance with the hospice plan of care.

Qualifications: Graduate degree preferred (Master's Divinity or Theology preferred), or Bachelor's degree with coursework from an accredited seminary or school of theology, with certification in hospital or pastoral ministry preferred. Minimum two years pastoral experience in a hospital or community ministry. Demonstrated ability to be open, flexible and ecumenical. Experience counseling patients and families who are dealing with terminal illness. Demonstrated understanding of hospice philosophy and principles. Demonstrated ability to be self-directed, flexible and cooperative in fulfilling role obligation, and ability to work effectively within an interdisciplinary team. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and in good working order.

 

Social Worker, Kaplan Family Hospice House (Per Diem, 4:30p-9p)
As a member of the Hospice House interdisciplinary team, the social worker reports to the Hospice House Manager , and is responsible for planning and providing supportive services to terminally ill patients and families. The social worker is responsible for identifying patient/family psychosocial needs, and for addressing those needs through direct service and/or consultation with other staff members in accordance with the hospice plan of care.

Qualifications: Master's degree in social work or related field preferred. Current Massachusetts social work licensure (LICSW preferred, LCSW accepted with appropriate work experience). Minimum three years counseling experience. Experience counseling terminally ill persons and their families; knowledge of the health care delivery system and community resources. Experience and training in leading support groups, and skill in the area of family psychosocial assessment. Demonstrated ability to assess and respond to the needs of patients and families in varied settings. Demonstrated understanding of hospice philosophy and principles.

 

Volunteer Coordinator (Full-time, M-F, 8:00 am-4:30 pm w occasional evenings)
Reporting to the Volunteer Manager, the Coordinator is responsible for assisting with the logistics of the Volunteer Program, including applications and screening, record management, recruitment, training, assigning and supervising volunteers.

Qualifications: Associate's degree in related discipline such as social work, psychology, education, human services, or business. Bachelor's degree preferred. Three to five years of program administration or office management experience in a healthcare or non-profit setting, preferably in a volunteer program or having served as a volunteer in a non-profit agency. Proven written and oral communication skills.

Reporting to the Vice President/CFO, the Business Analyst is responsible for the reporting and analysis of financial and operational statistics and data for Hospice of the North Shore & Greater Boston in order to assist in strategic and analytical decision making in all areas of the organization.  Develops, maintains and analyzes multiple reports from multiple systems in an executive level format that is clear and actionable.

Qualifications: Bachelor's degree in a quantitative discipline (Computer Science, Mathematics, Statistics, etc.) or Accounting, Finance, Business Management, Economics degree with analytical reporting experience. Master's degree and/or CPA a plus. Three to five plus years relevant experience. Advanced proficiency with Excel (Formulas, Graphs, Pivot tables, etc), Word and PowerPoint. Knowledge of Access, SQL and Relational Databases a plus. Ability to learn various software applications required for financial and operational reporting and analysis.
